Topamax, also known as topiramate, is a drug used to treat epilepsy and seizures. Topamax, manufactured by Ortho-McNeil Pharmaceuticals, has been prescribed to over 800,000 people since its introduction in the late 1990s.

Recently, Ortho-McNeil issued a warning regarding Topamax's tendency to cause eye disorders such as secondary angle closure glaucoma and acute myopia, conditions characterized by blurred vision and eye pain. According to a September 26, 2001 press release, at least 23 Topamax patients have developed glaucoma. While Ortho-McNeil reports that the condition is reversible and dissipates upon discontinuation of Topamax, failure to act at the first sign of these side effects can lead to permanent and serious vision problems.

In July 2003, Ortho-McNeil and FDA revised the WARNINGS and PRECAUTIONS sections of the prescribing information to provide updated information about oligohidrosis (decreased sweating) and hyperthermia, which have been reported in topiramate-treated patients. Oligohidrosis and hyperthermia may have potentially serious sequelae, which may be preventable by prompt recognition of symptoms and appropriate treatment.

Reports have primarily involved children. Most cases have occurred in association with exposure to elevated environmental temperatures and/or vigorous activity, and children should be observed closely under these conditions.

In December 2003, Ortho-McNeil and FDA revised the WARNINGS and PRECAUTIONS sections of the prescribing information, notifying healthcare professionals that Topamax causes hyperchloremic, non-anion gap metabolic acidosis (decreased serum bicarbonate). Measurement of baseline and periodic serum bicarbonate during topiramate treatment is recommended.
